We understand that suitable housing with the right support is an essential part of overcoming mental health challenges. Our range of mental health support services include:

The amount of support we provide varies depending on needs, and can be anything from a few hours a week, up to full-time support or intensive support over a short period of time to help achieve a goal.

We also encourage peer support as much as possible, where mutual support and a shared journey empowers people to make the changes and learn together.  Our services support individuals by encouraging them to focus on their strengths and build positive support networks to achieve their aspirations, whether that is returning to education, finding meaningful work or making new friendships.

Mental health is something that we can all relate to and something we provide support for in all services. Specialist mental health services support people with a diagnosed mental health condition, which can be anything from:

  • Anxiety and panic disorders
  • Bipolar disorder
  • Depression
  • Personality disorders
  • Schizophrenia

Through self-development and collaborative support with staff, the people we support gain self-confidence and improved emotional well-being on their pathway to independence and recovery.

We can help individuals to:

  • Manage medication and engage with mental health treatment programmes.
  • Establish and successfully maintain a tenancy.

  • Identify and apply for suitable move-on accommodation.
  • Build motivation, confidence and self-esteem.
  • Manage personal finances including claiming benefits, budget planning, paying bills and developing money management skills.
  • Access education, training, volunteering or employment.
  • Live a full and active life in the local community.

 

Our Approach to Personalised Support

Our approach to person-centered-Support is based around the use of an extensive personal-support-plans. The person-centered-support-plan is integral to providing a fully optimised set of support plans which set out a detailed and easy to follow plan of support. The plans are broken down into subsections depending on the service users’ individual needs and preferences. This allows for systematic detailed reviews on a monthly-basis and individual amendments and changes as soon as the need is identified.

All service-users have a new support-plan which is written based on information gathered from an Assessment (which ascertains whether an individual’s needs can be met from an organisational perspective), Talking with the service user themselves, liaising with family or carers, professionals and also analysing any specific medical notes. This information allows a set of support plans to be drafted as soon as a service user is taken on by us.

The Recovery Star tool, which was recommended by the Department of Health New Horizons programme (2009), has been developed for use in adult services. As a key-working tool it enables staff to support individuals they work with to understand their recovery and plot their progress. As an outcomes tool it enables organisations to measure and assess the effectiveness of the services they deliver.

Safeguarding

We have a robust Safeguarding policy which reflects that duties apply under the Care Act 2014.  The Care and support statutory guidance states that ‘all workers need to be vigilant about adult safeguarding concerns’ and that safeguarding practice should be underpinned by the six safeguarding principles:

  • Empowerment – people being supported and encouraged to make their own decisions and informed consent.
  • Prevention – it is better to take action before harm occurs.
  • Proportionality – the least intrusive response appropriate to the risk presented.
  • Protection – support and representation for those in greatest need.
  • Partnership – local solutions through services working with their communities. Communities have a part to play in preventing, detecting and reporting neglect and abuse.
  • Accountability – accountability and transparency in safeguarding practice.

 

Restorative Practice

To us, Restorative Practice means strengthening relationships between people as well as strengthening social connections within communities. Restorative practice describes a way of behaving. It involves building and maintaining healthy relationships, resolving difficulties and repairing harm if there has been conflict between individuals or groups.  We focus on building relationships with individuals and their families and connecting with the communities in which they live. We believe this will help us to better understand their needs, ensure we are empowering them to make positive changes and enable them to secure sustainable outcomes. We are committed to embed restorative practice as a fundamental part of the support we give. It is a common-sense approach.
